Athletics Clearance

Steps for Student-Athlete Clearance

1. Create an account for every sport your child will participate in for the 2021-22 school year at Athletic Clearance. 
   • Must have medical insurance & provide name and ID or policy number on medical card
   • Must provide the students’ doctor’s name and phone number
   • Watch this video for more information and further instructions.
2. Student athletes are required to complete a sports physical.
   • Download, print, and complete the forms below: 
     • Preparticipation Physical Exam Form
     • Preparticipation Medical History Form
       • *Reminder: ALL forms must be signed with a pen, not typed!*
   • Take the form to a physician to get a sports physical
   • Once completed, upload the completed physical to the student's Athletic Clearance account.
3. Download, Print, and Complete the Parent Consent Form
   • Parental Consent and Liability Release Form
   • Once you complete the steps, alert your Coach, Coach Prum and Mr. Lee 
4. Transportation Form (Optional)
