Address Verification

Address Verification: Utility bill, rental or lease agreement

• If renting a room within a residence and bills are not under your name:
  • Provide a utility bill for the residence under the landlord’s name
  • Landlord MUST sign the Residence Verification Form
  • Provide a second piece of mail showing parent’s name and address of residence

*Important* Have all necessary documentation with you when enrolling your student including birth certificate, immunizations, and any other important documents.

Parent/Guardian Documentation

• Copy of Parent/Guardian ID 
• If Applicable: Related court documents
• Caregiver Authorization Affidavit Form: If student lives with a relative or guardian

Requesting Transcripts

Education Verifications/School Records:

Former Students Requesting Transcripts/Records: You have two options:

• Stop by the school to fill out a transcript request form
• Fill out the digital transcript request form linked here

Please allow 5-7 business days for processing. You will be contacted when the transcript is ready to be picked up.
